In-Home & Travel Support

NDIS In-Home and Travel Support Services

Crown Again Care Connect provides flexible in-home and community-based support that helps participants develop practical, everyday skills in the environments where they feel most comfortable. Our therapists work with individuals in their homes, neighbourhoods, workplaces, and community settings to build confidence, safety, and independence.

In-home support may include daily living skill-building, emotional regulation strategies, routine development, sensory support, home safety assessments, and functional task practice. Travel support helps participants navigate the community safely — from public transport training to accessing appointments, shops, and recreation.

We tailor each session to the participant’s goals, strengths, and environment, ensuring support is respectful, person-centred, and aligned with NDIS priorities.

Frequently Asked Questions

What is included in NDIS In-Home Support?

In-home Occupational Therapy helps participants build functional daily living skills within their own environment. This may include practising routines, improving home safety, developing cooking or self-care skills, establishing structure, building independence, and implementing sensory or behavioural strategies at home.

Do you provide travel and transport support?

Yes. We support participants with community access related to their goals, such as attending appointments, therapy, shopping, community activities, and essential outings. Support may include travel training, road-safety practice, and building confidence navigating the community.

Can I choose my therapist?

Where possible, yes. We aim to match participants with therapists based on communication style, cultural needs, personality, and preferred routines to ensure comfort and consistency.

What funding does this come under in my NDIS plan?

In-home and community-based Occupational Therapy is typically funded under Capacity Building – Improved Daily Living, depending on the participant’s goals and plan type. Travel may be claimed under the NDIS travel rules.

Do you offer weekend or after-hours appointments?

Yes — appointments may be arranged for weekends, evenings, or public holidays depending on therapist availability. NDIS after-hours loading may apply.
