Referral Disclaimer

Crown Again Care Connect (“we”, “our”, “the Provider”) is committed to handling all referral information with the highest standards of privacy, confidentiality, and security. By submitting a referral form or providing referral information to us by any means, the referrer (“you”) acknowledges and agrees to the terms outlined in this Referral Disclaimer.
1. Purpose of Referral Information
Referral information is collected solely for the purpose of assessing eligibility, determining service needs, facilitating contact with the participant, and delivering NDIS-funded or private allied-health support. Information will not be used for any unrelated purpose.
2. Information We Collect in a Referral
The referral may include:
- Participant details (name, DOB, contact information)
- Diagnosis, disability-related information, medical or behavioural notes
- NDIS plan details and funding categories
- Support requirements, risk alerts, or safety considerations
- Referrer contact information and professional details
3. Permission & Authority to Share Information
By submitting a referral, you confirm that:
- You have obtained the participant’s informed consent (or guardian’s consent) to share their information with us; or
- You are legally authorised to provide this information on behalf of the participant.
We rely on the referrer to ensure consent has been properly obtained. Crown Again Care Connect is not responsible for incorrect, outdated, or unauthorised referral submissions.
4. Secure Handling of Referral Information
We take privacy seriously and follow strict security protocols. All referral information is:
- transmitted securely via encrypted platforms (where supported)
- stored in secure clinical systems with restricted staff access
- protected under Australian Privacy Principles and the NDIS Practice Standards
- never sold, shared, or transferred to third parties for marketing
5. How Referral Information Is Used
We use referral information to:
- review service suitability and clinical risk
- contact the participant, guardian, or support coordinator
- create or update client records
- organise appointments, assessments or service agreements
- liaise with professionals involved in the participant’s care (with consent)
6. Verification & Accuracy
We may contact the referrer to clarify information, verify details, or confirm service needs. We are not responsible for delays caused by incomplete, inaccurate, or missing referral information.
7. No Guarantee of Service Acceptance
Submitting a referral does **not** guarantee intake or service approval. Acceptance depends on:
- service capacity
- clinical suitability
- risk level
- NDIS funding availability
- geographical coverage
If we cannot accept the referral, the referrer or participant will be notified.
8. Sharing Information After Referral
With participant consent, we may communicate with:
- Support coordinators or plan managers
- Healthcare professionals
- Schools, residential facilities, or relevant support agencies
- NDIS or the NDIS Commission (if legally required)
Without consent, information will only be shared if legally required or necessary to prevent serious harm.
9. Storage, Retention & Disposal
Referral data becomes part of the participant’s confidential clinical record. Records are stored securely and retained according to:
- NDIS Practice Standards
- Australian privacy laws
- Allied-health regulatory guidelines
Once retention periods expire, records are destroyed securely and irreversibly.
10. Withdrawal of Referral
The referrer or participant may request withdrawal or correction of referral information. However, if services have already commenced, clinical notes and legal records cannot be deleted.
11. Liability & Responsibility
Crown Again Care Connect is not liable for:
- incorrect or outdated information provided by the referrer
- unauthorised disclosure made by the referrer before submission
- delays caused by missing or inaccurate details
